Who will submit the completed medical form to MOM?

At Truecare Clinic, our dedicated clinic staff will take care of submitting the 6ME medical form and the accompanying laboratory results directly to MOM on your behalf.

You will receive an SMS notification containing the submission reference number for your records. If you wish to keep a physical copy of the medical form, you can easily collect it from our clinic. In case you choose not to collect it, rest assured that we will securely discard the forms after a period of 2 weeks. Our aim is to provide a seamless and efficient process for your convenience.

What if my Helper needs to undergo a Chest X-Ray?

If the medical screening form from Truecare Clinic indicates that a Chest X-Ray is necessary, your Helper will be guided to visit the closest partnering Imaging Center to carry out the X-Ray. During the screening visit at Truecare Clinic, our staff will assist in scheduling the appointment for the X-Ray. Please be aware that transportation expenses to the partnering centers are not covered in the screening package.

Can I reschedule my appointment date at Truecare Clinic?

Certainly! At Truecare Clinic, you have the option to change the date and time of your appointment, subject to availability of slots. It is important to ensure that the health screening is conducted at least 2 weeks prior to the due date on the 6ME form. This precaution allows a safe timeframe for any necessary follow-up tests, such as repeat blood tests or chest x-rays, in case there are abnormal results that require further evaluation.

What documents do I need to prepare for the Pre-placement screening at Truecare Clinic?

For the Pre-placement Screening of Foreign Domestic Workers (FDWs) at Truecare Clinic, you will need to bring the following documents:
